Integrated Behavioral Health Services at Rock River Community Clinic

Behavioral health services encompass a range of support tailored to your unique needs. Through our integrated behavioral health model, our behavioral health (BH) provider and case manager act as your primary access point to connect you with the right services.

Integrated behavioral health at our clinic focuses on providing trauma-informed care, assessing behavioral health conditions, and collaborating with specialized providers and consultation programs, such as the Wisconsin Child Psychiatry Consultation Program. Our approach also includes strengthening motivational interviewing skills and managing medications for both psychiatric conditions and substance use disorders.

With integrated behavioral healthcare, we can offer treatment for conditions like depression, anxiety, and substance misuse within a primary care setting. Our behavioral health provider is available to see you within a week, or sometimes even on the same day, to help you find the most effective way to manage your condition. Additionally, our BH prescriber is on-site to provide medication management for various concerns.

If a different approach is better suited for you, our BH consultant and case manager can refer you to an external provider. We also offer Behavioral Health Integration (BHI) services, which may include follow-up calls from a nurse to monitor new medications, assistance with scheduling appointments, arranging transportation, and coordinating other needed services to help you achieve your mental health goals.

No insurance? No problem. Like many of our patients, you may instead qualify for our Sliding Fee Discount Program to receive care from a provider for as little as $10 per visit.
