Care Coordination Program at Rock River Community Clinic
At RRCC, our Patient Advocates, Referrals Team, and Prescription Assistance Program Coordinator work together to ensure the care you need is attained. They identify and address barriers that may impact a patient’s ability to manage their health, such as social needs, housing, transportation, and access to speciality medical services.
Our team provides education and connects individuals with the appropriate services to meet their unique needs. Coordinated care involves collaboration between the patient, their caregivers or families, community resources, their physician, and other healthcare team members as necessary.
