Sliding Fee Discount Program

The Sliding Fee Discount Program (SFDP) is based upon services rendered and has four-discount pay classes between 100% and 200% or less of the Federal Poverty Guidelines (FPG).

To apply, please schedule an appointment with a Care Navigator in either Whitewater, Watertown, or Fort Atkinson. You will need to bring identification that includes name and birth date, and proof of income for each household member over the age of 18. Household members are related by blood, marriage or adoption, and legally financially responsible to each other. 

Acceptable Documentation Includes:

  • Most recent pay stubs covering at least one month or more
  • Payroll and earnings verification statement or
  • Most current tax statements (1040) or W-2’s  (will only accept previous year)
  • One month of bank statements showing income deposits

If you receive any of the following, you will also need to bring a copy of your annual benefit(s) letter or one month’s worth of checks:

  • Social security
  • Disability
  • Pension
  • Trust Funds
  • Unemployment
  • Annuities

Your Care Navigator will work with you to verify your earnings and complete your application. All patients without insurance are financially screened prior to making an appointment.
