Care Coordination Program at Rock River Community Clinic

At RRCC, our Care Navigator, Financial Advocate, Referral Coordinator, and PAP Coordinator work together to assist in caring for patients outside of the medical appointment. They help to assess barriers that may be affecting one’s ability to manage their health, such as social needs, housing, transportation, safe environments, and access to certain medical services.

Our team provides education and coordinates extension (or referral) services to individuals that address their identified needs. Coordinated care involves the individual, their caregivers/families, community resources, their physician and other healthcare team members as needed.

Services may include:

  • Referral to behavioral health counseling
  • Referral to other community resources and programs
  • Assistance in establishing eligibility to Federal, State and local programs including, but not limited to:
    • Food assistance programs
    • Medicaid/Medicare applications
    • Medication assistance
    • Transportation to and from medical or dental appointments
    • Housing needs
  • Assistance in establishing eligibility for financial assistance for prescription medications.
