At RRCC, our Care Navigator, Financial Advocate, and Referral Coordinator work together to assist in caring for patients outside of the medical appointment. They help to assess barriers that may be affecting one’s ability to manage their health, such as social needs, housing, transportation, safe environments, and access to certain medical services.
Our team provides education and coordinates extension (or referral) services to individuals that address identified needs. Our coordinated care involves the individual, their caregivers/families, community resources, and their physician and other healthcare team members.
RRCC understands that physical and mental health go hand-in-hand and we are very committed to providing behavioral health services to our patients. As we are in the process of actively recruiting behavioral health providers to join our team, we want to help our patients with their behavioral health needs. To that end, we have established relationships with several healthcare systems, counties and private practice organizations who have expertise in this area and who will accept referrals from RRCC.
RRCC also has providers who can work with patients on certain preventative care services and self management support such as nutrition education, blood pressure monitoring, how to use a glucometer and other health equipment.
